VKORC1 makes the protein targeted by warfarin. The promoter SNP rs9923231 changes VKORC1 expression and contributes progressively to warfarin sensitivity. It is a direct single-SNP pharmacogenomic input, not a metabolizer star allele, and it must be combined with CYP2C9, clinical factors, and INR monitoring.
